Community Outreach Questionnaire

If your community has made improvements in its fire-protection services, you may be eligible for a PPCTM resurvey. If you'd like ISO to consider your city, town, or jurisdiction for a resurvey, download and complete the Community Outreach Questionnaire. The download is a PDF file, which you may view or print with the free Adobe® Acrobat® Reader.

When you have completed the questionnaire, mail it to your ISO Regional Processing Center.
